If a claim is reduced or denied

Ask for the specific policy language relied on, in writing. The most common reasons are a pre-existing determination drawn from an old record note, and a line item classified as routine. Both are appealable, and a letter from the treating vet explaining the clinical context is what usually moves them.

Before an expensive procedure

Ask the insurer for pre-authorisation. It is not binding everywhere, but it converts an argument-after into an answer-before, and it puts the reasoning in writing while you still have choices.
